The New Jersey Department of Health and Senior Services, Vaccine Preventable Disease (VPD) Program is announcing via this message that the American Recovery and Reinvestment Act (ARRA) Vaccine Program will open March 29, 2010, 8 AM, to March 31, 2010, 5 PM, for enrollment of providers who wish to participate. Enrollment is open to all medical providers who can utilize the vaccines. There are up to three hundred (300) openings available. Therefore acceptance will be of those who submit completed enrollment documents. Enrollment will end when that number is reached and representation from each county in the state is ensured. See statements below for complete information.
The VPD Program received $7,438,204 dollars in a Stimulus fund award in 2009. To achieve the greatest benefit for the citizens of NJ, a decision was made to utilize the funds to purchase 156,000 doses of vaccine that can be administered to any child, adolescent or adult regardless of insurance or non-insurance status. The only exception applies to individuals who are eligible to receive NJ Vaccines For Children (VFC) Program vaccine. They must continue to be vaccinated with the vaccines provided by that program.
The six (6) vaccines that will be available under this program are, Gardasil (HPV), Pneumovax 23, (Pneumococcal Conjugate) Prevnar 13, Adacel (TDaP), Boostrix (TDaP), and Zostavax.
Enrollment for selection to receive the vaccines for administration will begin on March 29, 2010. Due to the limited amount of vaccine available, the program will accept up to 300 locations that submit completed enrollment documents to the NJ VFC Program.
If you are interested in taking advantage of this significant opportunity to raise the immunization coverage levels in New Jersey, visit www.njiis.nj.gov to obtain the following enrollment forms for the program(s) of your choice. Click on VFC Vaccines For Children and look for the two page ARRA Vaccine Program Forms.
For NJ VFC ARRA Children’s Program (age birth to 18 years old), complete and submit:
· Provider Enrollment (Form 1)
· Provider Profile (Form 2)
· Additional Enrollment for Others Providers Within the Practice (IMM-26A) (if needed)
· Include a brief statement on your office’s letterhead indicating that you want to utilize your facility to receive and administer immunizations to any child who is not eligible to receive VFC vaccine and is requesting it, by using ARRA fund purchased vaccines.
For NJ VFC ARRA Adult Program (age 19 and up), complete and submit:
· Provider Profile Adults (Form 1)
· Provider Profile (Form 2)
· Additional Enrollment for Other Providers Within the Practice (IMM-26A) (if needed)
· Include a brief statement on your office's letterhead indicating that you want to utilize your facility to receive and administer immunizations to any adult that requests it, including health care workers, using ARRA fund purchased vaccines.
Submit the completed forms by fax to (609) 826-4868. DO NOT mail a copy of the forms as this will delay the review of your enrollment documents. Currently enrolled NJ VFC providers who wish to participate must also enroll in the ARRA program and be in full compliance with existing NJ VFC Program requirements. Upon receipt your application will be numbered, reviewed for completeness, and posted on the internal master index. You will receive a call from the NJ VFC Program to verify your facilities vaccine storage and handling capability.
All providers have the responsibility to account for each dose of vaccine received and used under ARRA. Failure to adhere to the accountability requirements will result in disenrollment from the ARRA funded vaccine program. All vaccine doses that are administered must be entered into the New Jersey Immunization Information System (NJIIS) or your practice management system for upload to the NJIIS prior to placing subsequent orders for vaccine. On-line training will be available for those who are not familiar with entering patient vaccination information into the NJIIS. Information regarding how to sign up for the training will be provided via email at the email address on your enrollment documents.
The program will end when all of the purchased vaccines have been distributed or will close midnight August 31, 2010 which ever occurs first, unless additional funding is provided to continue the program.
